Why Shoulder Pain Is More Complex Than It Looks

The shoulder is the most mobile joint in the body, which also makes it one of the most vulnerable. It relies on a careful balance of muscles, tendons, ligaments, and the surrounding joints working together. When any part of that system is off, the whole shoulder can suffer.

What makes shoulder pain tricky is that the source is not always in the shoulder itself. Problems in the cervical spine or upper thoracic spine can refer pain directly into the shoulder and arm, mimicking a local injury. That’s why a thorough evaluation matters before jumping to any conclusions about what is actually going on.

Signs Your Shoulder Needs Attention

Shoulder pain presents differently depending on the cause. Some people deal with a constant ache. Others feel fine at rest but have sharp pain with certain movements. Come in if you’re experiencing any of the following:

Pain or stiffness that limits your range of motion

 Aching that gets worse at night or disrupts your sleep

 Weakness when lifting your arm or reaching overhead

 A clicking, popping, or grinding sensation in the joint

 Pain that radiates down the arm or into the neck

If the pain has been lingering for more than a week or two, or if it came on after an injury, it is worth getting properly evaluated. Shoulder problems that go untreated tend to worsen over time, not improve.

Common Causes of Shoulder Pain

There are quite a few structures in and around the shoulder that can cause pain when irritated or damaged. The most common causes we see at Caruso Family Chiropractic include:

 Rotator cuff strain or injury. The rotator cuff is a group of four muscles that stabilize the shoulder. Overuse, repetitive motion, or a sudden injury can strain or tear these muscles, leading to significant pain and weakness.

Frozen shoulder. Also called adhesive capsulitis, this condition causes the shoulder joint to stiffen progressively, eventually making movement very limited and painful.

Shoulder impingement. When the rotator cuff tendons get pinched between the bones of the shoulder during movement, causing pain and inflammation, especially when reaching overhead.

 Cervical spine issues. Disc herniation or misalignment in the neck can send referred pain into the shoulder and arm, which is often mistaken for a shoulder problem.

 Muscle tension and postural imbalances. Poor posture, especially from desk work or prolonged phone use, creates chronic tension in the neck, upper back, and shoulders that builds into persistent pain.

 Bursitis. Inflammation of the bursa sac that cushions the shoulder joint, often caused by repetitive movements or direct impact.

How We Treat Shoulder Pain

Depending on what we find, your care plan may include:

Chiropractic adjustments. Targeted adjustments to the cervical and thoracic spine to address any referred pain coming from the neck, and to the shoulder joint itself when appropriate.

Soft tissue therapy. Hands-on work to release tight muscles and tendons around the shoulder, reduce inflammation, and restore normal tissue flexibility.

Shoulder mobilization. Gentle, guided movements to improve range of motion and break through stiffness without aggravating the joint.

Therapeutic exercises. Specific strengthening and stretching exercises to support the rotator cuff, improve stability, and prevent the problem from returning.

Posture correction. Identifying and addressing the postural habits that are putting ongoing strain on the shoulder and upper back.
